The ASCRS and ASOA Combined Ophthalmic Symposium is an exceptional educational opportunity. In an effort to have every member of your practice attend, ASCRS and ASOA offer special promotions and assistance. Registration Categories

Ophthalmologist: MD, DO, PhD
Fee includes badge, printed program, breakfast, lunch and breaks on Saturday and Sunday, and entrance to educational sessions and wet labs. Food and beverage will be served in the exhibit hall during all session breaks.

Young Eye Surgeon Resident/Fellow
MD or DO who is currently within an ophthalmic residency or fellowship program. A letter of verification with dates of residency/fellowship is required to finalize registration within this category.

Fee includes badge, printed program, breakfast, lunch and breaks on Saturday and Sunday, and entrance to educational sessions and wet labs. Food and beverage will be served in the exhibit hall during all session breaks.

Shared hotel accommodations at the JW Marriott will be provided for residents and fellows, traveling from outside the greater Austin, TX area (100 mile radius). Please note that hotel rooms are based on double-occupancy shared accommodations. ASCRS will be contacting you by June 28 to collect the necessary information to confirm your hotel room, including asking for roommate requests. We will attempt to honor roommate requests and encourage you to communicate with your preferred roommate, so requests match up. If you do not request a specific roommate, one will be assigned. ASCRS will make the hotel reservation for you and there is no need to book direct with the JW Marriott. Alternatively, if you decline the shared hotel accommodations, you may qualify for a $250 stipend. In order to qualify for the shared hotel accommodations or travel stipend, you must be present for the entire ASCRS Young Eye Surgeons Program. Young Eye Surgeon in Practice
MD or DO who is currently within their first five years of practice.

Fee includes badge, printed program, breakfast, lunch and breaks on Saturday and Sunday, and entrance to educational sessions and wet labs. Food and beverage will be served in the exhibit hall during all session breaks.

Military MD, PhD or DO
Reduced pricing is offered to members of the United States military. In order to obtain this rate, registrant must have an active military email address. Please ensure your military email address is used for your registration record to avoid any adjustments in fees.

Practice Management Staff
Practice management staff personnel. Physicians may not register in this category.

Fee includes badge, printed program, Friday evening welcome reception, entrance to all Friday and Saturday educational sessions, as well as breakfast, lunch, and breaks both days. Food and beverage will be served in the exhibit hall during all session breaks.

Optometrist
Fee includes badge, printed program, breakfast, lunch and breaks on Saturday, breakfast and breaks on Sunday, and entrance to all educational sessions. Food and beverage will be served in the exhibit hall during all session breaks.

Corporate Personnel
Manufacturers or other corporate personnel who are not exhibiting and who wish to attend the meeting. Solicitation is not permitted.

Fee includes badge, printed program, breakfast, lunch and breaks on Saturday, breakfast and breaks on Sunday, and entrance to all educational sessions. Food and beverage will be served in the exhibit hall during all session breaks.

Nurses & Technicians
Registration for the Nurses and Technicians program track is handled directly by the American Society of Ophthalmic Registered Nurses (ASORN). Please visit the ASORN website for full registration details.

Payment

Payment (in U.S. funds) can be made by personal check, bank draft, travelers check, AmEx, VISA, or MasterCard. No other credit cards will be accepted. Payments by check will be accepted through August 9, 2019. Credit card payments will be accepted through August 20, 2019. (You must register on site after this date.)

Cancellation/Refund Policy

All cancellations and requests for refunds must be in writing and received no later than July 26, 2019. A handling fee of $75 will be deducted from each cancelled registration. NO REFUNDS will be given after July 26.

Photo/Video Sharing Terms and Conditions

By attending the ASCRS ASOA Combined Ophthalmic Symposium, you agree that your name and likeness may be used by ASCRS, ASOA in associated marketing and promotional materials.

ADA/Special Accommodations

ASCRS fully comply with the legal requirements of the Americans with Disabilities Act (ADA) and the rules and regulations thereof. ASCRS also requires ADA compliance from contracted vendors and facilities.
