**Letter of verification with dates of residency/fellowship is required. A $300 travel reimbursement is available for registrants.  All US residents and fellows with verified attendance will receive the stipend.

Registration Categories

Ophthalmologist: MD, PhD, or DO
Fee includes badge, printed program, breakfast, lunch, Friday night welcome reception and breaks on Saturday, breakfast and breaks on Sunday, and entrance to all educational sessions. Food and beverage will be served in the exhibit hall during all session breaks.

*Military MD, PhD or DO
Reduced pricing is offered to members of the United States military. In order to obtain this rate, registrant must have an active military email address. Please ensure your military email address is used for your registration record to avoid any adjustments in fees.

Resident/Fellow
MD or DO who is currently within an ophthalmic residency or fellowship program. A letter of verification with dates of residency/fellowship is required to finalize registration within this category.

Fee includes badge, printed program, breakfast, lunch and breaks on Friday, entrance to educational sessions and wet labs on Friday. Complimentary registration to Ophthalmologist’s program educational sessions Saturday and Sunday, breakfast, lunch, and breaks Saturday, breakfast, and breaks on Sunday. Food and beverage will be served in the exhibit hall during all session breaks.

Practice Management Staff
Practice management staff personnel. Physicians may not register in this category.

Fee includes badge, printed program, Friday evening welcome reception, entrance to all Friday and Saturday educational sessions, as well as breakfast, lunch, and breaks both days. Food and beverage will be served in the exhibit hall during all session breaks.

Optometrist
Fee includes badge, printed program, breakfast, lunch and breaks on Saturday, breakfast and breaks on Sunday, and entrance to all educational sessions. Food and beverage will be served in the exhibit hall during all session breaks.

Corporate Personnel
Manufacturers or other corporate personnel who are not exhibiting and who wish to attend the meeting. Solicitation is not permitted.

Nurses & Technicians
Registration for the Nurses and Technicians program track is handled directly by the American Society of Ophthalmic Registered Nurses (ASORN). Please visit the ASORN website for full registration details.

Payment

Payment (in U.S. funds) can be made by personal check, bank draft, travelers check, AmEx, VISA, or MasterCard. No other credit cards will be accepted. Payments by check will be accepted through August 21, 2018. Credit card payments will be accepted through August 21, 2018. (You must register on site after this date.)

Cancellation/Refund Policy

All cancellations and requests for refunds must be in writing and received no later than August 1, 2018. A handling fee of $75 will be deducted from each cancelled registration. NO REFUNDS will be given after August 1.
